How to Become a Resident

Save this checklist

  • Contact our Sales & Marketing Department at 802-985-9847 to arrange for an appointment and site tour.
  • Obtain a Priority Number by completing the attached Wait List Agreement and submitting a deposit of $2,500.
  • Complete the Application for Residency packet,including Financial and Medical Information forms, which will be mailed to you a few months prior to move-in.
  • Submit the Release for Medical Information form to your physician(s).
  • Follow up with your physician(s) to be sure the Release for Medical Information form is completed and mailed to The Lodge at Shelburne Bay.
  • Schedule an appointment for a medical assessment at The Lodge at Shelburne Bay.
  • Review Residency Agreement and Resident Handbook and write down any questions.
  • Upon financial and medical approval, set up an appointment with the Director of Sales & Marketing to sign Residency Agreements. At this time, the security deposit and first month’s rent are due.
